We are glad you have chosen LLAD for our child's education.  We need to gather some information from you and your child so we can best meet his/ her needs.  Please complete the Application Form first.  You can either email it to us at Peggy.Key@deafacademy.com or to

Once you have completed the application form, please have your student complete the Student Questionaire.  The Parent Questionaire needs to be completed by the parent/ guardian.  We prefer both parents/ guardians complete the form together.  The completed forms can either be emailed or mailed as described above.

When the LLAD staff receives these forms, we will process them and then contact you for a meeting.  The final step in the application process is an interview of the parent and an interview of the student.
